For patients with HFrEF (like EF 20%), the combination drug Sacubitril/Valsartan (often known by the brand name Entresto) is considered a revolutionary and essential medication. It often replaces traditional ACE inhibitors or ARBs and is part of the "quad-therapy" regimen.


What is Sacubitril/Valsartan?

This drug is a unique combination known as an Angiotensin Receptor-Neprilysin Inhibitor (ARNI). It combines two active ingredients:

  1. Valsartan: An Angiotensin Receptor Blocker (ARB).

  2. Sacubitril: A Neprilysin Inhibitor.

The Science: A Dual Action Strategy

  • Valsartan's Role: It blocks the effects of a harmful hormone called angiotensin II, which normally causes blood vessels to narrow and the heart to stiffen. By blocking it, Valsartan helps lower blood pressure and eases the heart's workload.

  • Sacubitril's Role: It blocks an enzyme called neprilysin. Blocking neprilysin increases the levels of natural substances in your body (natriuretic peptides) that are beneficial for the heart. These beneficial substances help:

    • Relax blood vessels.

    • Increase the excretion of salt and water (like a mild diuretic).

    • Inhibit harmful heart remodelling.

The result is a medication that not only eases stress on the heart but also actively promotes better heart function.

Why is ARNI Crucial for Heart Failure (LVSD)?

The key benefit of Sacubitril/Valsartan is its proven ability to reduce hospitalisations and improve survival compared to older heart failure drugs.

  2. High Potassium (Hyperkalemia): ARNIs, like Eplerenone (which you also take), can increase potassium levels. Your specialist nurse will order frequent blood tests to monitor your kidney function and potassium levels.

  3. Angioedema: This is a rare but serious side effect involving rapid swelling of the face, lips, tongue, or throat. If this occurs, seek emergency medical help immediately (call 999 in the UK). If you were previously taking an ACE inhibitor (like Ramipril), there is a required 36-hour "washout" period before starting Sacubitril/Valsartan to minimize this risk.

Key Advice: This medication is often started at a very low dose and slowly increased (titrated) over weeks or months to find the optimal dose for you. Patience and close monitoring are essential for success.
